Minutes Of The Meeting

Ohio State Board of Pharmacy

December 23, 2003

 

TUESDAY, DECember 23, 2003

 

  1:30 p.m.

ROLL CALL

 

 

Pursuant to Section 4729.16(D) of the Revised Code, the State Board of Pharmacy con­vened a telephone conference call for the purpose of considering a summary suspension as required by Section 3719.121 of the Revised Code with the following members partici­pating:

 

Robert P. Giacalone, R.Ph. (President); Lawrence J. Kost, R.Ph. (Vice-President); Diane C. Adelman, R.Ph.; Gregory Braylock, R.Ph.; Suzanne R. Eastman, R.Ph.; Elizabeth I. Gregg, R.Ph.; Nathan S. Lipsyc, R.Ph.; and Dorothy S. Teater, Public Mem­ber.

 

Also participating were Timothy Benedict, Assistant Executive Director; David Rowland, Legal Affairs Administrator; and Sally Ann Steuk, Assistant Attorney General.

R-2004-104

After hearing Mr. Benedict discuss the significant facts regarding the activities of the indi­vidual, Mrs. Gregg moved that the Board summarily suspend the license to practice phar­macy belonging to Tim Ivan Miller, R.Ph. (03-3-11708) due to the fact that a continuation of his professional practice presents a danger of immediate and serious harm to others.  The motion was seconded by Mr. Lipsyc and approved by a roll call vote of the Board members participating (Aye-7/Nay-0).

After hearing Mr. Benedict discuss the significant facts regarding the activities of the in­dividual, Mrs. Teater moved that the Board summarily suspend the license to practice pharmacy belonging to Zachary Daniel Hennekes, R.Ph. (03-2-23584) due to the fact that he has admitted to an addiction to controlled substances and due to the fact that a con­tinuation of his professional practice presents a danger of immediate and serious harm to others.  The motion was seconded by Mr. Braylock and approved by a roll call vote of the Board members participating (Aye-7/Nay-0).

After hearing Mr. Benedict discuss the significant facts regarding the activities of the indi­vidual, Mrs. Gregg moved that the Board summarily suspend the license to practice phar­macy belonging to James Joseph Parsley, R.Ph. (03-1-24212) due to the fact that he has admitted to an addiction to controlled substances and due to the fact that a continuation of his professional practice presents a danger of immediate and serious harm to others.  The motion was seconded by Mrs. Adelman and approved by a roll call vote of the Board members participating (Aye-7/Nay-0).

The business of the meeting thus finished, the telephone conference call was ended.
